Job Description

Case Manager

Location: Oakland, CA
Schedule: Monday to Friday, 9:00 AM – 5:00 PM
Salary Range: $100,000 – $120,000 per year

Job Summary:
We’re hiring a full-time Case Manager to assist individuals with service access, resource coordination, and support planning. In this office-based role, you’ll meet with clients, develop care strategies, and ensure that all services are delivered efficiently. You’ll also maintain strong documentation and guide clients through each step of their plan.

Duties:

  • Assess client needs and create care plans
  • Coordinate service providers and referrals
  • Maintain detailed and accurate records
  • Track client progress and update care as needed
  • Facilitate communication between parties

Requirements

  • Bachelor’s in Social Work, Psychology, Nursing, or related field
  • 2+ years in a case management or similar support role
  • Clear communicator with strong time-management skills
  • Knowledge of local community networks and providers
  • Licensure (LCSW, LMSW, RN) preferred but not required
