Case Manager

Location: Berkeley, CA
Schedule: Monday to Friday, 9:00 AM – 5:00 PM
Salary Range: $100,000 – $120,000 per year

Job Summary:
We’re bringing on a Case Manager to help clients connect to services and maintain stability. This office-based role focuses on client advocacy, resource coordination, and supportive care planning. You’ll work closely with individuals to help them access the tools and services they need, while monitoring progress over time.

Duties:

  • Design and implement care plans tailored to each person
  • Coordinate appointments, referrals, and service providers
  • Maintain accurate and timely records
  • Serve as liaison with healthcare professionals and agencies
  • Regularly review and revise care plans based on outcomes

Requirements

  • Bachelor’s in Social Work, Psychology, Nursing, or related field
  • 2+ years in a case management or similar support role
  • Clear communicator with strong time-management skills
  • Knowledge of local community networks and providers
  • Licensure (LCSW, LMSW, RN) preferred but not required
